Forms and leads issues
Use this guide when form submissions or quiz leads are missing, delayed, duplicated, or not reaching integrations.
First checks​
- Confirm the page or quiz is published.
- Submit a fresh test using realistic data.
- Check the correct lead area for the form or quiz type.
- Remove filters from the leads list.
- Confirm required fields were completed.
- Check whether an integration is expected to receive the lead.
Lead appears in Atomicat but not in an integration​
If the lead exists in Atomicat, the form captured correctly. The issue is likely with the integration, webhook, or external destination.
Check:
- Webhook URL or integration settings.
- Required destination fields.
- Whether the receiving system rejected the submission.
- Whether the integration account is still connected.

Lead does not appear in Atomicat​
If Atomicat does not show the lead:
- Confirm you submitted the published page, not only a builder preview.
- Check required fields.
- Test on another browser.
- Disable browser extensions that block forms or scripts.
- Republish after changing form settings.
Exports look incomplete​
Exports reflect the current filters and lead type. Clear filters, confirm date range, and make sure you are exporting from the correct list.
